Major Insurance Plans Accepted
We accept several health insurance plans. Please Note: some plans do not cover and pay for medical nutrition therapy with a Licensed & Registered Dietitian. Thus, please consult your health insurance company or policy for nutrition benefit details and find out:
- If you have Medical Nutrition Therapy/Nutrition Counseling benefits with a Licensed and Registered Dietitian
- How many Visits are allowed per Calendar Year or benefit contract period
- If you are responsible for Copays or Deductible costs
- If you need an Electronic Referral or Doctor’s Script from your Primary Care Physician/Family Doctor
Referrals
If your health insurance plan requires Referrals from your Primary Care Physician, please obtain the referrals before each Nutrition Counseling appointment. You can provide those referrals to us in one of two ways:
- Bring the paper referral document with you to your appointment, or
- Ask your doctor’s office to send an electronic referral directly to us. Also, it may be helpful to request Electronic Referrals from your doctor for multiple visits in advance to avoid delays/rescheduling of your Nutrition Counseling appointments.
Doctor's Scripts
If your health insurance plan requires Doctor’s Scripts from your Primary Care Physician, please obtain the doctor’s scripts before each Nutrition Counseling appointment. You can provide those doctor’s scripts to us in one of two ways:
- Bring the paper script document with you to your appointment, or
- Ask your doctor’s office to fax a copy of the script directly to us.
Please Note: Referrals and Doctor's Scripts can have an expiration date and/or a limited number of allowed visits. If the expiration date has passed or the number of allowed visits has been met, please obtain a new Referral or Doctor's Script before appointments made after the expiration date or after all allowed visits have been used.
We are in-network with and accept health insurance from the following health insurance organizations:
